About This Role

The Holistic Support Worker is under the supervision and direction of Mental Health Clinical Manager and the Cultural Manager, will provide support to individuals, families, and community at large at various levels of service delivery through programming with therapeutic and culturally based perspectives.

Responsibilities

✓• Participate in the weekly case management team meetings and monthly case supervision. • Work as a team player by collaborating with colleagues to conduct traditional/clinical assessments and develop treatment plans based on experience and knowledge. • Provide mentoring to all age groups in collaboration with the clinical staff. • Monitor client(s) by doing regular home visits, wellness checks, and/or office appointments. • Assist with providing assessments, case planning, and individual treatment plans for each client. • Carry a caseload and perform administrative duties such as updating case notes using the Agency’s program, maintain file upkeep, upload documents, fill out necessary forms as required. • Function as a liaison and/or advocate to other mainstream services providers where deemed necessary. • Ensure that both traditional and modern treatment modalities are provided to the client(s). • To work with community Knowledge Keepers, Cultural Advisors, Elders who are able to provide sound traditional and ceremonial teachings that would support the Agency’s Service Delivery.
